Traveling to Oklahoma City for Surgery: A Plan for Phoenix Patients
Phoenix is about 836 miles from our Oklahoma City practice — a flight or a long drive. Patients make the trip because they have found a specific surgeon, a specific procedure focus, or a facility standard that fits their goals.

Why patients travel from Phoenix to Oklahoma City
Cosmetic surgery is elective. You choose the surgeon, the facility, and the city. Some patients find the right match close to home. Others look further — across state lines, across the country. The decision usually comes down to a specific surgeon's experience with a specific procedure, or the standards of the facility where the surgery will be performed.
Oklahoma Surgical Arts operates out of an AAAHC-accredited surgery center in Oklahoma City with two operating rooms. Dr. Angelo Cuzalina founded the practice and directs its AACS-accredited fellowship. His published case volume exceeds 18,000 procedures, with particular focus on rhinoplasty, deep plane facelift, complex revision surgery, and total-body contouring. The six cosmetic surgeons of the Tulsa Surgical Arts roster all serve Oklahoma City patients, and Dr. Trent Fogleman practices here full-time.
Phoenix patients follow the same path every patient follows. You meet the surgeon. You are examined. You describe what bothers you in your own words. You leave with a written plan and a written, all-in number — both yours to keep, to decide on your own time. There is no pressure in the room and none after you leave it.
Starting with a virtual consultation
Every plan here begins the same way, whether you live seven miles away or 836. Virtual consultations let Phoenix patients have a thorough initial conversation without booking a flight. You can share your goals, ask questions, and learn whether the surgeon you are considering is the right fit — all before committing to travel.
Ask for a specific surgeon when you book, or share your goals and the team will match you. You choose your surgeon and your city — surgery happens in Oklahoma City or Tulsa. When surgery is scheduled, a single in-person pre-operative visit confirms the plan. Many traveling patients combine that visit with their surgery trip.
Getting here and knowing what to bring
The practice is located at 12400 Saint Andrew's Dr, Oklahoma City, OK 73120. Parking is free and on-site. On surgery day, your driver pulls directly to the entrance for drop-off and pick-up. The team confirms your arrival time for your specific case — not a general clinic opening.
Bring photo ID, your completed paperwork, a written list of every medication and supplement you take, glasses and a case — no contact lenses. Wear loose, front-opening clothing and slip-on shoes. No makeup, lotion, deodorant, or fragrance on surgery day. Leave jewelry, piercings, and valuables at home or at your hotel.
After anesthesia you may not drive yourself, take a rideshare alone, or leave unaccompanied. A responsible adult drives you and stays with you for the first 24 hours. If you are traveling from Phoenix, plan for that companion to be with you through your initial recovery period in Oklahoma City.
Surgery day and the journey home
You arrive at the time confirmed by the surgical team. Check-in, pre-operative preparation, and a final review with your surgeon and anesthesia provider happen before you go back. You wake in recovery under one-to-one observation, then discharge home with your driver and your written instructions.
Your surgeon's team will tell you when it is safe to fly or drive back to Phoenix. When any instruction on this page differs from what your surgical team tells you directly, their instructions win. Follow-up appointments can often be coordinated with virtual check-ins, reducing the number of return trips.
The practice phone is 405-751-0042. If you have questions at any point — before, during, or after your trip — the team is reachable. Patient forms, including registration in English and Spanish, are published as PDFs so you can print, fill out, and bring them with you.
The full menu of procedures
Breast augmentation is this practice's flagship — alongside the full menu: tummy tuck, liposuction, breast lift, eyelid surgery, rhinoplasty, mommy makeover, gynecomastia surgery, and facial surgery. Dr. Cuzalina's particular focus areas include rhinoplasty, deep plane facelift, complex revision surgery, and total-body contouring.
The practice also offers injectable treatments — BOTOX®, Jeuveau™, Dysport, and XEOMIN®; Juvéderm, SkinVive, Radiesse, Sculptra, and Versa — along with physician-dispensed skincare. These are delivered on the program's standards: consultation first, never a cart. For Phoenix patients considering a combination of surgical and non-surgical care, the team can coordinate a plan during your visit.
