Traveling to Oklahoma City for Surgery: A Plan for Salt Lake City Patients
Salt Lake City is about 856 miles from our Oklahoma City practice — a flight or a long drive. Patients make the trip because they choose a specific surgeon, a specific facility, and a specific standard of care.

Why patients travel for cosmetic surgery
Cosmetic surgery is elective. You choose the surgeon, the facility, and the city. Some patients find the right match close to home. Others look further — across state lines, across the country. The decision usually comes down to a specific surgeon's experience with a specific procedure, or the standards of the facility where the surgery will be performed.
Oklahoma Surgical Arts operates out of an AAAHC-accredited surgery center in Oklahoma City with two operating rooms. Dr. Angelo Cuzalina founded the practice and directs its AACS-accredited fellowship. His published case volume exceeds 18,000 procedures, with particular focus on rhinoplasty, deep plane facelift, complex revision surgery, and total-body contouring. The six cosmetic surgeons of the Tulsa Surgical Arts roster all serve Oklahoma City patients, and Dr. Trent Fogleman practices in Oklahoma City full-time.
The consultation starts the plan
Every plan here begins the same way, whether you live seven miles away or 856. You meet the surgeon. You are examined. You describe what bothers you in your own words. You leave with a written plan and a written, all-in number — both yours to keep, to decide on your own time. There is no pressure in the room and none after you leave it.
Consultations are available in person at the Oklahoma City office or virtual from anywhere. Virtual consultations let Salt Lake City patients have a thorough initial conversation without booking a flight. When surgery is scheduled, a single in-person pre-operative visit confirms the plan. Ask for a specific surgeon when you book, or share your goals and the team will match you.
You choose your surgeon and your city — surgery happens in Oklahoma City or Tulsa. The practice phone number is 405-751-0042.
Getting here and knowing what to bring
The practice is located at 12400 Saint Andrew's Dr, Oklahoma City, OK 73120. Parking is free and on-site. On surgery day, your driver pulls directly to the entrance for drop-off and pick-up. The team confirms your arrival time for your specific case — not a general clinic opening.
Bring photo ID, your completed paperwork, a written list of every medication and supplement you take, glasses and a case (no contact lenses), phone and charger. Wear loose, front-opening clothing and slip-on shoes. No makeup, lotion, deodorant, or fragrance on surgery day. Leave jewelry, piercings, and valuables at home.
After anesthesia you may not drive yourself, take a rideshare alone, or leave unaccompanied. A responsible adult drives you and stays with you for the first 24 hours. Your driver checks in with you, stays reachable by phone for the entire day, and receives your discharge instructions in person.
Surgery day and recovery
You arrive at the time confirmed by the surgical team. Check-in, pre-operative preparation, and a final review with your surgeon and anesthesia provider follow. You wake in recovery under one-to-one observation, then discharge home with your driver and your written instructions.
If you are traveling from Salt Lake City, plan your stay so you can attend any required follow-up appointments before heading home. When any instruction on our resource pages differs from what your surgical team tells you directly, their instructions win. The Tulsa flagship has a private overnight guest suite — your consult, records, and photography follow you between cities if needed.
